Histopathologic examination of hearts from individuals with the diagnosis of ACM have revealed contractile protein loss, fragmentation, and disarray, supporting the concept of altered protein physiology/composition (11, 29,31). Alcoholic cardiomyopathy is characterized by dilation and impaired contraction of one or both myocardial ventricles.

This test will assess the ejection fraction (EF), a measurement that expresses how much blood the LV pumps out with each contraction. Finally, it should be noted that McKenna and co-workers, in one of the most frequently cited papers in the ACM field, reported an incidence of 40% in 100 individuals suffering from idiopathic DCM, but in this case the consumption threshold used was only g/d[8]. At present ACM is considered a specific disease both by the European Society of Cardiology (ESC) and by the American Heart Association (AHA)[18,19]. Patients who consume more than two drinks per day have a 1.5- to 2-fold increase in hypertension compared with persons who do not drink alcohol, and this effect is most prominent when the daily intake of alcohol exceeds five drinks. Because hypertension may directly contribute to left ventricular (LV) dysfunction, this may be a confounding comorbidity in persons who abuse alcohol, and it should be differentiated from pure forms of alcoholic cardiomyopathy. Among the many ethanol and heart studies, mitochondrial dysfunction or evidence of impaired bioenergetics has been a common finding. This is exemplified by either a change in mitochondrial ultrastructure and/or depressed indices of bioenergetics and oxidative phosphorylation.

In contrast, at the other end of the drinking spectrum, recent large population-based studies have revealed possible protective effects of light to moderate consumption of alcohol against heart failure. There was an approximate halving of the hazard ratio for congestive heart failure among drinking men (8–14 drinks per week) and women (3–7 drinks per week) in the Framingham Heart Study (Walsh et al., 2002) compared to subjects who consumed less than 1 drink per week.
